Undo stops working

The undo option stops working all the time on me. Anyone see this?

press undo browser button (between undo and redo) and report the topmost entry there. This entry has jammed and collects undo data inside rather than let it accumulate in a list. Is it ZM3 issue or ZM2 issue?
